Please note ALL Patients are required by the State to see their physician every 7 months for a follow up visit in order to keep their certification and prescriptions.

ALL Patients are required by the State to Renew their MMUR CARD every 12 months . (Each Card has an annual expiration date printed on it) Annual renewals are done through each patient’s online MMUR Profile with login info.

Walk-ins are available on marked follow up days between 9am and 4pm
